Providence Health & Services Service Excellence Rep - Hospice Continuity of Care Center in Torrance, California
Providence is calling a Service Excellence Representative Continuity of Care Center (Full time/Day shift) at Providence TrinityCare Hospice in Torrance CA.
This is an 8-hour shift position, 9:00am-5:30pm. Must be able to work at least 1 Saturday each month.

In this position you will:
Work in a very busy environment and provide information and resources to the inquiring customer.
Exercise considerable initiative and judgment while providing excellent customer service.
Follow-up and follow-through are imperative attributes. This person has a thorough knowledge of services provided throughout the Providence continuum and acts as a resource to the community.
Work in collaboration with other members of the Call Center to: obtain insurance verification and/or authorizations for patients at time of admission, schedule patient/family presentations or admission appointments, verifying that all admission documentation present per agency and regulatory requirements.
Document ongoing conversations with referral partners, patients/families, hospice liaisons, Admission RN, Triage RN, and/or other hospice staff per protocols within the EHR. Responsible for entry of accurate ICD codes for the hospice primary, secondary and/or related diagnosis into the patient record.
Valid driver's license and automobile insurance in accordance with state and/or PH&S/Trinity Care Hospice requirements
Excellent phone skills, good oral and written communication skills, good word processing and computer skills, and able to operate office equipment. Is able to use Excel software
Has strong organizational skills and is detail oriented
Bilingual in Spanish
Experience in a related call center environment, physician office, or clinic
Knowledge of community resources available for patients and families
