Providence is calling an Intake Nurse (RN) (Part time/Day Shift) to Providence TrinityCare Hospice in Torrance, CA.
We are seeking an Intake Nurse (RN) who will act as a contact for information and arrangement of all TrinityCare Hospice services from physicians, HMOs, discharge planners and other outside referral sources. The Intake Nurse will provide access for new patients to TrinityCare Hospice services.
In this position you will:
Receive case referrals from physicians, HMO insurers, discharge planners, etc., ordering hospice services:
For home care referrals, takes hospice evaluation and admission order and assures appropriate staff are notified to follow up that all services and products are arranged
Follow up with referrals to confirm that all services and products have been arranged
For home care referrals take orders and assure appropriate people are notified and follow up with referrals to confirm that all services and products have been arranged
Assess patient appropriateness for admission based on intake data and admission criteria
Provide information on TrinityCare Hospice services to physicians, HMOs, insurance companies, discharge planners and other referral sources
Assess customer service needs and assure linkage with appropriate department and program
Maintain productivity standards as set by organization guidelines
Documentation / Information Management:
Assign medical record number and log referral according to office protoco
Enter initial referral information into computer system for patient tracking
Adhere to current TrinityCare Hospice policies and contractual arrangements
Adhere to record keeping system for managing telephone intake services
Collect data on callers’ requests for information and services
Leadership / Supervision:
Participate in orientation, education and development of team members as directed by supervisor
Participate in appropriate continuing education as may be requested and/or required by immediate supervisor
Accept personal responsibility for other educational activities to enhance job-related skills and abilities
Receive grievances, problems, and complaints from callers and follow up with appropriate departments to assure satisfactory resolution. Add to log of complaints, noting department and nature of complaint
Monitor current and potential referral sources for feedback on TrinityCare Hospice’s performance
Participate in the Performance Improvement Program and participates in and adheres to the LCMHS/TrinityCare Hospice Corporate Integrity Plan

Required qualifications for this position include:
Nursing, graduate of an NLN approved school
Current unencumbered California RN license
Possesses and maintains current American Heart Association BLS Certification for Healthcare Providers
Valid driver's license with an automobile that is in good working order and insured in accordance with state and/or LCMHS/TrinityCare Hospice requirements
Minimum 1-year RN experience in hospice, palliative care, home health, acute care &/or skilled nursing
Demonstrates knowledge of home care products and services
Demonstrates effective and diplomatic telephone communication skills
Demonstrates a working knowledge of computer applications including Microsoft Office and e-mail
