We are seeking a Clinical Pharmacist who will responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. The Clinical Pharmacist will serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team. The clinical pharmacist; 1) assures safe and effective medication use and; 2) demonstrates proficiency in delivering care to assigned age specific patient population (e.g., infants, adolescents, adults, geriatrics, and pediatrics); and 3) participates in related continuing education.
In this position you will:
Promote patient, physician, customer and employee satisfaction by personal example, treating all contacts with respect
Maintains required applicable licensure, certification and education
Demonstrate knowledge of age specific care, including but not limited to, identifying the need for additional safety measures, or activities that place patients at risk including knowledge of age appropriate dosage ranges, IV admixtures, and special equipment, able to perform age appropriate dose-check calculations. Communicates effectively in a clear, concise, understandable manner. Applicable for the populations: Infant, Pediatric, Adult, and Geriatric
Perform computer order entry
Comply with hospital formulary requirements
Participate in maintenance of pharmacy inventory, purchasing supplies and medications
Perform role as supervisor to the pharmacy technician
Demonstrate willingness & ability to float to areas within their scope of practice as necessary.
Implement and maintains established department, unit, and hospital policies, procedures, and objectives as well as improving organization performance program, safety, environmental, and infection control policies
Actively participate in daily rounds with physicians to identify patients for therapeutic monitoring and to discuss potential interventions
Coordinate ongoing quality assessments and improvements of clinical pharmacy activities in collaboration with physicians and implements necessary action as appropriate
Coordinate and/or provides comprehensive clinical pharmacy services to patients and may include drug therapy assessment based upon evaluation and interpretation of patient history, age, medical condition, surgical history and objective physical biochemical data; development of therapeutic goals and endpoints of drug regimen; preparation of discharge orders and patient education within scope of practice
Educate and effectively collaborate with Pharmacy Staff to ensure safe, legal, and effective drug distribution and use. Monitors drug therapy and suggest alternate drug therapy when appropriate
Prepare clear and concise clinical briefs for use by nurses, medical staff and pharmacy
Effectively communicates verbally and/or in writing clinical pharmacy functions to the interdisciplinary team
Serve as a drug information resource person to physicians, patients and other health care staff
Participate in educational activities and programs for the interdisciplinary team
Act as a liaison with nursing and medical staff concerning prescriptions, orders, drug and pharmaceutical detail, patient reactions, errors and complaints
Consult with and advises medical staff personnel regarding utilization of drugs and pharmaceuticals
Maintain competency and enhances professional growth and development through participation in educational programs, current literature, in service meetings, and workshops
Attend meetings and participates on committees as required
Demonstrate a comprehensive clinical pharmacy knowledge base
Serve as a preceptor to pharmacy residents and students when applicable
Required qualifications for this position include:
PharmD from an Accredited College of Pharmacy
Bachelor's Degree pharmacy-Acceptable for employees hired prior to the revision date of this Job Description (10/01/12)
Current California Registered Pharmacist’s license in good standing
Current LA City Fire Card within 30 days of hire
Preferred qualifications for this position include:
- 2 years acute care experience or a Pharmacy Residency equivalent to 3 years of pharmacy practice experience
