Providence Lead Bereavement Coordinator in Torrance, California
Providence is calling a Lead Bereavement Coordinator (Full time/Day shift) at Providence TrinityCare Hospice in Torrance, CA.
We are seeking a Lead Bereavement Coordinator with significant clinical skill to manage high risk and complicated grief. The Lead Coordinator wil provide bereavement services to individuals which include assessments, treatment plans, information and resources, grief education, crisis intervention, bereavement counseling and support group facilitation. Outreach and education are also provided to the community. The Lead Coordinator may provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
In this position you will have the following responsibilities:
Assess the psychosocial status of individuals related to their adjustment to transition and loss. Develop a Bereavement Plan of Care for the identified bereaved according to standards of clinical practice and evaluated plan for ongoing appropriateness and implementation. Provide bereavement counseling to individuals and families
Perform quality assurance on the clinical documentation of the Bereavement Team Plan to ensure Bereavement Plans of Care are complete and accurate
Perform suicide/homicide assessments of high risk individuals and providing appropriate follow-up
Assist with the planning, coordination and facilitation of bereavement groups and facilitates support groups in a skilled and compassionate manner
Maintain relationships and serve as a point of contact with local Master of Social Work programs and coordinate placement of Master of Social Work interns at Providence TrinityCare Hospice
Provide clinical supervision to the Bereavement Team and Master of Social Work interns, as directed. Participate in orientation, education and development of team members and volunteers as directed by supervisor
Provide clinical supervision to Program Coordinator of The Gathering Place and signs off on clinical documentation
Assume responsibilities and communication between Bereavement Team and the Administration
Complete initial intakes and follow-up assessments in a timely manner and provide appropriate resources and referrals based on assessment
Submit clinical and administrative documentation within the timeframe set by the organization guidelines
Perform other duties as requested by Administrator of Senior and Community Services
Required qualifications for this position include:
Master's Degree in Social Work; program accredited by the Council on Social Work
Must be a California licensed driver with an automobile that is in good working order and insured in accordance with state and/or Providence TrinityCare Hospice requirements
2 years post license experience in the field of social work
3 years experience in the area of bereavement and loss
1 year experience in Hospice Care
Demonstrates knowledge of bereavement and loss
Demonstrates good verbal and written communication skills and organizational skills
Ability to work as a team member
Preferred qualifications for this position include:
Associate Clinical Social Worker [ACSW] working toward License to practice Social Work in the state of California
Licensed to practice Clinical Social Work in the state of California
Possesses and maintains current CPR certification
1 year experience in support group facilitation
1 year experience as a supervisor/preceptor with Masters in Social Work interns
1 year experience in working with volunteers
Bilingual in Spanish
Demonstrates working knowledge of computer applications, i.e. Microsoft Office and e-mail
