Providence Health & Services Hospice Community Liaison - San Fernando Valley Area in Torrance, California

Providence is calling a full-time, day shift Hospice Community Liaison to Providence TrinityCare Hospice in Torrance, CA.

This position will be based at Providence Tarzana Medical Center and will cover the surrounding San Fernando Valley area.

In this Position you will:

  • Develop marketing tactics and strategies to implement action plans with Managers and Planning Groups, Case Managers / Care Coordinators, Physicians, Hospitals and Skilled Nursing Facilities to ensure continuity of care and service in the assigned service areas.

  • The goal is to improve customer service, increase referrals, establish and maintain relationships with referral sources, facilitate patient admissions, identify opportunities for improvement, evaluate the effectiveness of one's own activities through data collection integration and analysis, provide education both internally and externally, participate in related targeted professional activities, and foster staff responsibility for customer service.

Qualifications:

Required qualifications for this position include:

  • Minimum 3 years experience / responsibility in hospice, home health or skilled nursing facilities, with knowledge to market, evaluate patients, problem solve and educate on hospice theory and practice

  • Valid driver's license with an automobile that is in good working order and insured in accordance with state and / or PH&S / Providence TrinityCare Hospice requirements

Preferred qualification for this position include:

  • RN or LVN preferred

  • Demonstrated strong micro / personal computer experience

  • Proficiency with Microsoft Office applications and capable of mastering new software applications in a timely manner

  • Ability to communicate clearly and effectively, make effective verbal presentations and write effective reports

  • Proven ability to be flexible and responsive to customer needs

  • Strong interpersonal skills and effectiveness in working well within a team environment

  • Ability to speak and understand both English and Spanish language(s) with the ability to comprehend and communicate effectively preferred.
