Providence Health & Services Diabetes Educator Certified RN in Torrance, California

Description:

Providence is calling a Diabetes Educator Certified RN (Full time/Day shift) for the Providence Medical Institute in Torrance, CA.

We are seeking a Diabetes Educator Certified RN who will serve as an educator and consultant to patients, families, caregivers, and providers in the prevention and management of diabetes. Provide one-on-one and group teaching of diabetes management to patients. Demonstrate proficiency in comprehensive patient assessment, delivery of personalized care, and customized education based on patient's needs (age-specific, culturally appropriate, pertinent to medical history, etc.).

In this position you will:

  • Serve as educator and consultant to patients, families, caregivers, and providers in the prevention and management of diabetes

  • Perform job functions timely and efficiently

  • Deliver upon the service expectations of both our patients and fellow caregivers by listening to their needs; engaging in positive interactions; and following through on promises made in a thoughtful, efficient, timely and courteous manner so that their total outcome is better than expected

  • Respect the dignity, confidentiality and privacy of patients

  • Work in a safe manner, adhering to general safety precautions and standards Report any unsafe conditions to their supervisor and/or the safety hotline

  • Provide patients with one-on-one and group teaching of diabetes management, inclusive of medication, blood sugar, meters, diet, complications and survival skills

  • Document patients’ progress in medical record. Consults with providers regarding patient management and communicates progress

  • Monitor compliance and management of diabetes patients by tracking patient data, i.e.,lab values of glycated hemoglobin levels and blood sugars

  • Work with providers and supports good diabetes management. Prepare, review and maintain current instructional materials on diabetes

  • Teach from PMI’s current and approved Education Curriculums

  • Provide technical guidance and direction in development of diabetes programs and protocols

  • Work with the policies and procedures of the appointment scheduling system for scheduling of patients

  • Assist with development of policies, procedures and programs

  • Actively participate in quality improvement and outcome studies

  • Participate as part of the clinical resource team to provide orientation and training of caregivers in diabetes related topics

  • Provide productivity reports to manager

  • Support and assist the clinical resource team in development and implementation of other programs, as needed

  • Assist with instruction of other patient education programs, as needed

  • Demonstrate the ability to work independently with minimal direction, adjusting to changes in workload and acuity

  • Follow correct procedures for handling and disposing of biohazardous infectious waste. Comply with infection control and safety procedures

  • Follow environmental cleaning and equipment quality control and maintenance guidelines

  • Order and maintain current inventories of diabetes education supplies

  • Perform inventory and check for outdated diabetes education supplies on a monthly basis

  • Use appropriate reporting mechanisms as required by federal and state laws

  • Obtain appropriate authorizations and consents for treatment

  • Assist patients in developing their own health program. Counsel patients and family on the importance of hygiene, exercise, medications, blood glucose monitoring, and nutrition in patient’s health and wellbeing

  • Provide open and timely communication with patients, families, caregivers, and providers

  • Provide sufficient information to allow patients and their families to participate in the patient’s care

  • Provide educational resources to the patients, families, caregivers, and providers, as appropriate

  • Demonstrate commitment to education and sharing of knowledge

  • Assist in the orientation of new caregivers

  • Obtain ongoing education consistent with level and area of practice

  • Maintain current American Heart Association BLS for Health Care Providers and other appropriate licensure/certifications

  • Participate in quality improvement audits annually

Qualifications:

Required qualifications for this position include:

  • Current California RN license

  • Certified Diabetes Educator certification

  • Current American Heart Association BLS for Health Care Providers

  • Must have PC knowledge in Microsoft Office

  • Possess excellent customer service and communication skills (both verbal and written)

  • Must have properly insured vehicle available at all times to be able to offer educational programs at all PMI sites and throughout the community

  • Must be willing to teach at multiple sites during daytime and evening hours, as needed

Preferred Qualifications:

  • Bachelor's Degree in Nursing

  • 2 years experience in diabetes management in an out-patient environment

  • Experience working with patients of all ages
