Providence is calling an RN Palliative Care Clinical Supervisor (full-time / day shift) at Providence TrinityCare based in Torrance, CA.

We are seeking a Palliative Care Clinical Supervisor who will be responsible for ensuring that patient and family care is coordinated, and delivered appropriately 24 hours/day, 7 days/week for palliative care services. The Clinical Supervisor, Patient Care Team is responsible for the day-to-day supervision of the team's clinical staff. The Clinical Supervisor, Patient Care Team ensures effective collaboration and communication with internal and external customers and healthcare delivery systems that impact the day-to-day care provision of care. The Clinical Supervisor, Palliative Care serves as a liaison to community resources and referral sources. The Palliative Care Team may provide care, primarily in complex case situations, to premature infants, infants, pediatrics, adolescents, young, middle and older adults. The Clinical Supervisor, Palliative Care will hold responsibility for Palliative Care clinical operations, including, but not limited to: oversight of IDT member scheduling for patient care, facilitation of IDT meetings, facilitation of morning "AM Huddles", working directly with patient/families on service failures/complaints, collaboration with other Clinical Supervisors or Clinical Managers on "sharing" staffing as needed, preparation of the weekend staffing schedules, weekend visit schedules, working in partnership with the After Hours Team and Call Center for seamless admissions and coordination of patient care, and assignment of patient caseloads to IDT members. The Clinical Supervisor, Palliative Care will share responsibility for the team's clinical operations along with the Regional Director, Palliative Care, including, but not limited to audits of client records (EMR and paper), documentation review and remedial action, tracking and follow-up with Quality Assurance Performance Improvement programs and reporting measures.
In this position you will:
In collaboration with the Regional Director, recruit, interview, hire and evaluate professional staff for Palliative Care Team
Participate in maintenance and oversight of training programs for Palliative Care services
Complete annual supervisory visits and competency evaluations on all direct reports
Manage expenses for Palliative Care clinical services to meet costs per day, and/or other budgetary expectations
Review and evaluate the documentation of each team clinician, three (3) months after hire and ongoing, using the information to mentor each team clinician to promote a high quality of documentation that is well supported for eligibility and reimbursement
Provide coaching and guidance to clinical staff, serves as a resource for information and clinical support
Collaborate with Clinical Managers and Supervisors, as needed, in support of patient care/services provided by Palliative Care Team
Maintain productivity standard as set by organization guidelines
Participate in, track and trend organizationally determined Quality Performance Measures
Periodically review and evaluate documentation of supervised staff, identify clinical and/or compliance issues and design remediation to address identified issues
Review documented results of Patient Care patient care outcomes, perform client chart audits and report results to supervisor within established agency protocols
Promote development and utilization of standardized Patient Care Protocols
Utilize EMR to analyze and trend performance measures
Leadership / Supervision:
Manage Patient Care Team clinical staff including; RNs, LVNs, Social Workers, Chaplains, and Certified Home Health Aides
Active participant in the developing, training and evaluating Palliative Care services and patient care outcomes achieved
Assist in coordinating Palliative Care Team regular staff meetings for information, support and education
Present and promotes Palliative Care Team services within Palliative Care at PTCH and the greater community
Actively collaborate with other Clinical Supervisors and Managers in coordinated delivery of patient care
Participate in appropriate continuing education as may be requested and/or required by immediate supervisor
Accept personal responsibility for other educational activities to enhance job-related skills and abilities
Attend mandatory educational programs
Participate in the review and revision of the organization’s policies, protocols and procedures as requested
Participate in the Performance Improvement Program and participates in and adheres to the PH&S Corporate Integrity Plan
Demonstrate a commitment to increasing efficiency and utilization of Call Center staff and services
Participate as needed in the budget preparation for program development
Promote cost-effective delivery of all services
Manage clinical labor expenses for Patient Care Team patient care team services to meet budgetary expectations
Required qualifications for this position include:
Associate's/Bachelor's/Master's Degree in Nursing or Master's Degree in Social Work or Spiritual Care
Current California professional licensure in the applicable discipline
Current driver's license and proof of automobile insurance
Minimum 2-years' experience in Adult or Pediatric hospice care, Adult or Pediatric Palliative Care, Adult or Pediatric oncology, or Long Term Care, or Professional Case Management or other end of life care associated field
Proven ability to work in an interdisciplinary setting
Demonstrates excellent computer literacy, including Microsoft applications (Outlook, etc.) and clinical documentation with Electronic Health Records
Demonstrates excellent verbal, written, and observation skills
Understands principles of pain/symptom management
Understands hospice philosophy, end of life care coordination and issues of death/dying
Preferred qualifications for this position include:
Bachelor's Degree in Nursing
Two (2) years supervisory experience in hospice, home health, case management, acute care, or long term care setting
Current LCSW licensure in the state of California or in process
