We are seeking a Clinical Pharmacist who will be responsible for ensuring the safe and appropriate use of drugs. Fulfillment of this responsibility is achieved through the provision of pharmaceutical services whose components include drug information, drug distribution and clinical pharmacy services with knowledge of age-specific needs from pediatrics through geriatrics.
In this position you will have the following responsibilities:
Leadership:
The following duties must be performed with routinely and daily assigned tasks must be completed by the end of the shift:
Monitor safe medication systems at all times
Perform or oversee the dispensing of controlled and non-controlled drugs to inpatients and outpatients
Perform or oversee the preparation of IV's, TPN (Adults and Neonates), chemotherapy, and other preparations for IV administration
Check of all medications prior to dispensing.
Patient education and counseling regarding drug therapy as needed
Provision of oral and written consultations with other health care professionals regarding drug therapy selection and management
Drug therapy monitoring and communicating relevant findings and recommendations to other health-care professionals who are also responsible for the patient's care: Drug therapy monitoring includes an assessment of:
Therapeutic appropriateness of drug regimen
Appropriateness of the route of administration
Drug-drug, drug-food, or drug-disease interactions
Screening for allergies
Duplication of therapy
Provision of Clinical Pharmcokinetics Services
Monitor, detect, document, and report of adverse drug reactions as per hospital policy
Assist with and/or supervises the planning, coordination and delivery of drug dispensing activities in team environment
Assign responsibility based on daily activity and patient volume
Seek assistance immediately if unable to solve the problem on own
Assist with orientation and preceptoring as assigned
Provide input to development and revision of policies and procedures
Promote harmonious relationships and favorable attitudes amongst the Health Care team
Documentation/information Management:
Demonstrate working knowledge of the Hospital Information System and Pharmacy component
Perform documentation in patient's chart which is timely, accurate and concise per departmental policies/procedure
Document Adverse Drug Reactions, Clinical Interventions, Compounding Log, and appropriate documentation on prescriptions in accordance with regulatory agencies and hospital standards
Comply with all regulatory agency requirements including State Board of Pharmacy, Department of Public Health, and The Joint Commission
Professional Growth/Development:
Assume responsibility and accountability for own professional growth and practice. This includes maintaining current licensor and completing all required in-service education with emphasis in primary area of work
Assist with staff development and acts as a resource to staff and ancillary support services
Attend assigned staff development activities for department as requested
Participate in in-services as requested
Serve on various committees as assigned
Seek validation of knowledge base, decision-making and skill level by questioning when necessary
Participate in drug use evaluations and other quality assurance programs
Review drugs for formulary consideration
Performance Improvement/Quality and Safety:
Participate in assigned performance improvement projects and works to correct deficiencies identified
Actively supports the facility with regards to continuous compliance with all regulatory and performance improvement standards including, but not limited to The Joint Commission (TJC), Department of Public Health, CMS Core Measures, IHI initiatives, etc.:
Maintain current knowledge regarding TJC standards for this position
Commit to and actively participates in ongoing compliance
Have knowledge of Tracer methodology
Provide the best care and service to every person, every time (PHS Quality Vision)
Provide care that is safe, timely, effective, efficient, equitable and patient/family centered (PHS Quality definition)
Stewardship:
Assist in controlling drug costs by recommending the most cost effective drug therapy
Promote departmental programs designed to reduce drug costs such as the therapeutic interchange programs
Practice financial responsibility and prudence
Comply with budget and staffing standards
Qualifications:
Required qualifications for this position include:
Current California Pharmacist license
Minimum of five (5) years recent experience in hospital pharmacy or a pharmacy residency
Previous experience with personal computers
Knowledge of sterile products techniques
Knowledge of personal computers.
Proficiency and current knowledge in pharmacy and therapeutics in 5 of the following areas at all times:
Antimicrobial therapy
Pain management
Pharmacokineties
Pediatrics
Cardiovascular drugs
Critical care medicine
Geriatrics
Neonatal
Diabetics
Oncology
