We are seeking a Clinical Liason who will be responsible for creating and maintaining positive customer relationships to increase referrals for Home Health, Hospice/Palliative Care, Transitional Care Units, and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, facilitate the coordination of services and to ensure appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and home health; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on Home Health, Hospice/Palliative Care or other issues in various settings and facilities. In this position you will:
Maintain and build existing referral base relationships by serving as a resource for education and information
Evaluate referrals received on hospitalized patient from a variety of care settings for appropriateness for Home Health, Hospice/Palliative Care, and/or other in-home services programs as appropriate
Provide information to patients and their families interested in Hospice, Home Health and/or Infusion services
Participate, when appropriate, in health care facility care conferences regarding specific patient consult requests
Facilitate the initiation of an Infusion referral as needed
Coordinate discharge planning for Hospice and Home Health patients who have been admitted to the hospital and act as a liaison to Ministry staff for smooth transition
Accept referrals from hospital discharge planner after they have identified patient agency choice and type of home service needed
Coordinate with the Inpatient Hospital Team and primary RN/MSW to ensure discharge planning is comprehensive and communicated efficiently
Collaborate in Care Plan development, implementation and evaluation with discharge planners. Participate, when appropriate, in discharge planning rounds in health care facilities
Appropriately document activities in the Providence electronic medical system; tracks referrals received by nursing unit and accepted by each Providence agency
Maintain open positive communication with appropriate agency staff (Hospice / Palliative Care, Home Health, and/or other in-home services) regarding referrals received
Effectively manage customer resolution issues independently or seeks management/leadership involvement as needed
Schedule, coordinate and present in-services and other information presentations at community events, health fairs, care-giver series, and other outreach activities
Coordinate with Account Representative to support referral generation efforts; facilitate growth of Home Health, Hospice and Palliative Care services and promote Providence Senior and Community Service Ministries according to business development strategies
Actively participate in the development and execution of strategic initiatives that include referral generation and business growth in assigned accounts
Comply with all applicable Federal, State, and local laws, regulations, and requirements as well as facility specific policies and procedures. In addition, promote the Patients’ Bill of Rights and maintains confidentiality of all patient demographics, medical and financial information at all times
Maintain confidentiality of all company and client information; ensure proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations
Respond to daily needs of local Team, including facilitation of excellent communication channels between Business Development and Intake Department
Serve as a local champion for service-line standardization and consistency efforts
Required qualifications for this position include:
Bachelor's Degree in Nursing, social work, or other clinical discipline Or Associate's degree plus 4 years related experience
Current California State professional licensure or certification, including RN, LVN, MSW or LCSW license, as required appropriate to education
Current California State Driver's license
Current BLS from the American Heart Association
Minimum 3-years' experience in clinical discipline
Ability to achieve annual referral and community educational requirements by prioritizing efforts and activities to support increased referral volume and achieve account penetration
Demonstrates history of successful clinical educational presentation skills, sales and marketing results, preferably in healthcare related field
Excellent verbal and written communications skills in English
Excellent customer service communication skills by phone, email, and face to face
Communicates clearly, sensitively and in supportive professional manner with patient/family/caregivers, co-workers and public
Ability to follow appropriate communications channels
Demonstrates competency by maintaining positive, collaborative, and constructive interpersonal relationships
Understands and practices the principles of effective teamwork
Applies and maintains strong team collaboration skills within and across ministries, and in particular, builds strong relations with operations and clinical team members
Ability to work with families in crisis

Demonstrates flexibility and responsiveness working within a dynamic environment
Proficient in using computers and knowledge of an Electronic Medical Record (as applicable) and Microsoft Office Suite programs
Experience and knowledge of customer relationship and territory management software
Ability to travel throughout the service area from site to site and access multi-level buildings
Self-directed with the ability to work with little supervision
Map reading skills, ability to drive safely and to follow directions in order to navigate effectively in varied road conditions and inclement weather
Preferred qualifications for this position include:
Liaison, marketing or sales experience
Health care industry experience
3-years' hospice or palliative care experience
