The Clinical Liaison will be responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Transitional Care Unit and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and home health; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on Home Health or other issues in various settings and facilities.
This position will be traveling throughout South Bay community with a focus at skilled nursing facilities/board and care facilities/ assisted living facilities and Providence ministries.

Education sufficient for licensure
Current OT, PT, ST, MSW, LPN, LVN, MA or RN licensure required in the state in which the caregiver resides
Current Driver's license
Current Healthcare Provider CPR certification
Bachelor's Degree in Nursing or other clinical discipline (e.g., OT, Social Work)
Three (3) years of experience in clinical discipline
Liaison, marketing or sales experience
Health care industry experience
Documented prior successful sales account medical management experience
Epic EMR experience
